Affiliate Recruiter Program Terms & Conditions

Effective: May 1, 2025

These Terms and Conditions ("Agreement") govern participation in the Care Coordination Staffing Affiliate Recruiter Program ("Program") operated by Care Coordination Staffing LLC ("Company", "we", "us"). By enrolling in the Program, you ("Affiliate") agree to comply with and be bound by this Agreement.

1. Eligibility

• Must be at least 18 years old.

• Must reside or be legally registered to do business in the United States.

• Must not be employed by or directly affiliated with a competing healthcare staffing company unless approved in writing.

2. Program Description

• Affiliates refer Registered Nurses (RN), Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA), or Geriatric Nursing Assistants (GNA) to work with Care Coordination Staffing.

• Commissions are earned for each qualified referral who:

o Successfully completes onboarding and credentialing

o Works a minimum number of hours in a facility as defined by the Company

3. Commission Plans (Choose One)

Plan A: Recurring Commission

• 4% of gross wages paid to referred staff

• Paid monthly for up to 9 months

• Requires referred staff to work at least 1 shift during that month

Plan B: One-Time Commission

• $500 for RN and LPN referrals

• $300 for CNA/GNA referrals

• Paid after the referred staff completes onboarding and works their first shift

Affiliates may only choose one plan per referral. Dual commission under both plans is not permitted.

4. Commission Payment Terms

• Payments made monthly via [ACH, PayPal, or method chosen by Company]

• Commissions processed within 15 business days after the end of the work month

• Affiliates must submit required tax documentation (e.g., W-9) before receiving payment

5. Qualified Referrals

A referral is considered qualified if:

• The candidate has not previously applied to or worked with the Company

• The candidate is successfully credentialed and completes at least one shift

• All documentation is complete and accurate

Duplicate or previously known candidates are not eligible for commission.

6. Affiliate Responsibilities

• Must use only approved marketing content or language

• Must not make misleading claims about compensation or job opportunities

• Must not submit false information or impersonate candidates

7. Program Changes and Termination

• The Company may change commission amounts, structure, or eligibility at any time with 7 days’ notice

• Either party may terminate this agreement at any time, with or without cause

• In case of fraud or breach of terms, the Company reserves the right to withhold unpaid commissions

8. Independent Contractor Relationship

Affiliates act as independent contractors. This Agreement does not create an employment, agency, or partnership relationship.

9. Limitation of Liability

Care Coordination Staffing is not liable for:

• Loss of income or opportunities due to candidate disqualification or non-participation

• Delays due to credentialing, onboarding, or compliance requirements

10. Governing Law

This Agreement shall be governed by the laws of the State of [Delaware, Maryland, or where the business is registered].

11. Acceptance

By signing up through the Partnero affiliate system or submitting your first referral, you agree to these terms in full.
